Lilly's Foundayo Wins US Approval for Oral GLP-1 Weight Loss Treatment
Eli Lilly & Co. has secured US approval for Foundayo (orforglipron), a groundbreaking oral GLP-1 drug for weight loss that can be taken once daily without food or water restrictions. This development has the potential to significantly expand the use of obesity medicines, which are currently dominated by injectable therapies.
Lilly plans to price Foundayo at as little as $25 a month for commercially insured patients and $149 for self-pay users at the lowest dose in the US market. This pricing strategy aims to position the drug for mass adoption and make it more accessible to a broader patient base.
The approval marks a significant milestone in the fast-growing GLP-1 market, where adoption has been limited by needle aversion, dosing complexity, and access barriers. Currently, fewer than one in 10 eligible patients use GLP-1 drugs, despite soaring demand and strong clinical data.
Foundayo, whose active ingredient is orforglipron, has been approved by the US Food and Drug Administration for adults with obesity or overweight individuals with weight-related conditions, when used alongside diet and physical activity. The drug will begin shipping in early April through LillyDirect, with wider pharmacy availability to follow.
In contrast to injectable GLP-1s such as Lilly's own Mounjaro and Novo Nordisk's Wegovy, Foundayo is a small-molecule pill that does not require cold-chain storage and has no fasting or timing constraints. This simplifies manufacturing and distribution, eases adherence, and makes treatment more acceptable in primary-care settings rather than specialist clinics.

The treatment also improved multiple cardiovascular risk markers, including cholesterol and blood pressure, reinforcing its role as a cardiometabolic therapy. In Lilly's ATTAIN-1 trial, patients on the highest dose of Foundayo lost an average of 27 pounds, or 12.4 percent of body weight, over 72 weeks.

The USFDA's decision sets the stage for oral GLP-1s to reshape obesity – and eventually diabetes – treatment globally, with filings underway in more than 40 countries.
