Wildfire Smoke Exposure Leads to Rare Respiratory Condition in 87-Year-Old Man

A recent case report published in the New England Journal of Medicine highlights the unusual connection between prolonged exposure to wildfire smoke and a rare respiratory condition known as plastic bronchitis.

The patient, an 87-year-old man, was admitted to an emergency department after experiencing shortness of breath following several hours of inhaling dense smoke from a forest fire. Doctors observed that he was taking shallow breaths and had significantly reduced blood oxygen levels, prompting immediate intervention. Due to the severity of his condition, he was placed on a ventilator while clinicians examined his airways.

During evaluation, medical staff identified unusual obstructions within his respiratory passages — thick, dark, branching structures lodged in the windpipe and lungs. These formations were diagnosed as bronchial casts, a defining feature of plastic bronchitis. Typically, bronchial casts are composed of mucus and cellular debris, with a rubber-like consistency and a white, tan or beige appearance. However, in this case, their darker appearance was associated with smoke exposure.

Plastic bronchitis is most often linked to congenital heart conditions in children or disorders affecting lymphatic circulation. The condition arises when the flow of lymphatic fluid becomes disrupted, allowing excess fluid to leak into the airways and contributing to the formation of casts. Under normal circumstances, the lymphatic system removes excess fluid from tissues and returns it to the bloodstream.

Symptoms of plastic bronchitis may resemble common respiratory disorders such as asthma, which can lead to delayed or incorrect diagnosis. In severe cases, complications can progress to respiratory failure and may be life-threatening. Diagnosis typically involves imaging and direct visualisation of the airways, including procedures such as bronchoscopy and chest CT scans.

Treatment strategies vary depending on the cause of the condition. These may include interventions to repair lymphatic leaks or specialised care if the condition is associated with cardiac abnormalities. In some cases, doctors employ a technique known as "blue bronchoscopy", where dye is introduced into the thoracic duct to identify points of leakage.
